Helpsite > Case Management > Consent for Transportation and Emergency Medical Treatment

• Once the document is signed
• Rename document to match policy

• ​Once logged in to Extended Reach>Click on the Client from your My Workspace

• Add New Report: “Consent for Med Transportation”
• Enter Date which matches the date on the document, or if no date on the form use the received date

• Click on the paperclip under Related Files

• Find and click Open to Upload Document
• Click Complete when finished
