Coordinated Care’s AHCC Overview. Apple Health Core Connections (AHCC) is Coordinated Care’s Integrated Managed Care Program for children and youth in foster care and adoption support as well as alumni of foster care (ages 18-26). The overview provides information on how to access and use covered physical and behavioral health services and added benefits including information about Teledoc.
