*After uploading a document is finished click on Complete or Submit for Approval NOT Save

Child Leaves CFSF Home
- Once logged in to Extended Reach>Click on the Client from your My Workspace
- Add Report>Child Leaves CFSF Home
- Change Submit to: “Staff, Fiscal”
- Add in relevant information
- Click Submit for Approval
- If rejected: you will receive an email, correct the Report and resubmit for approval
- If approved by the supervisor, an email automatically gets sent to kidmove@cfsf.net
Child Transfers to Another CFSF Home
- Once logged in to Extended Reach>Click on the Client from your My Workspace
- Add Report>Child Transfers to Another CFSF Home
- Change Submit to: “Staff, Fiscal”
- Add in relevant information
- Click Submit for Approval
- If rejected: you will receive an email, correct the Report and resubmit for approval
- If approved by the supervisor, an email automatically gets sent to kidmove@cfsf.net
- If CPA kid go to the last step (3. New Placement Made)
- If Non-CPA kid, confirm Program Type. Follow below for instructions:
- To see what the home is allowed to take Go to Home Rosters>Licensed Homes>Open Home
- Within the General tab confirm that the Custom Field>Program Type box is checked for the Program the child will be placed in; BRS, Borrowed Bed, ICPC, In Home Wraparound, Intensive Case Management, Receiving Care, or Receiving Care Tier 2
- If the box is checked go to the last Step (3. New Placement Made)
- If the box is not checked
- Confirm that the home has done the necessary training and any other necessary items to allow that program
- Scroll up and find the Licensor for the home
- Send the Licensor a message to add a check to the program type
- New Placement Made
- Delete Forms and Documents
- Send helpdesk@cfsf.net the URL of the item needed to be deleted.
- Social Worker – Add/Changes or needs contact info updated
- Contact updates@cfsf.net. Use the ExtendedReach ID number of the child and state the change needed
- Include: Full name, Email, Phone Number(s)
- If the contact info needs updating, be sure to state if the new information is in addition to what is there already or if it replaces the information that’s already there.

- Client Relationships – Add/Changes or needs contact info updated
- Contact updates@cfsf.net. Use the ExtendedReach ID number of the child and state the change needed
- Include: Full name, Email, Phone Number(s), and Relationship Type
- Relationship Types = Brother, CASA/GAL, Child’s Attorney, Daycare, Dentist, Doctor, Eye Doctor, Father, Foster Support Specialist, Grandparent, Medical Specialist, Mother, Primary Care Physician, Psychiatrist, Relative, School, Sibling, Sister, Social Worker, Stepfather, Stepmother, Teacher, Therapist, WISe

- Can’t see kid(s) on your caseload
- Contact updates@cfsf.net by email and let them know of the issue
- Covering for a co-worker
- If you are covering for someone and need access to a client’s file on that case managers case load you would contact updates@cfsf.net
- If sending an email please fill out the email like below:
- Subject: Filling in for case manager
- Body: Include the name of the employee you are filling in for and the date of when that employee returns to work.
- If you need something immediately contact an IT staff member.
- If Child on your caseload changes Programs (for example: CPA to BRS)
- Instead of re uploading documents to the new case file, Add report match dates from old case file and click Complete.
- Case Rosters>Foster Care Open Cases>Change Group Case
- Expand All reports
- Under Summaries are reports
Oversite List
- Open Extended Reach
- Go to Case Rosters>Foster Care Open Cases
- Go to Group Cases>Case Manager (For clinical directors: Go to Group Cases>by Age/Gender)
- Click the Plus button next to your name (For clinical directors: click Expand All)
- Click on Excel
- Open the Excel sheet that just downloaded
- Click Enable Editing
- Now format the excel sheet
- Delete columns you do not need but make sure to keep:
- Client Name
- Current Placement
- Level
- Admission
- Clinical Directors also keep: Column 1, Branch, Caseworker
- Oversite Reports
- Go to Home Rosters>Licensed Homes
- Group by>Custom Field>Expand Case Manager (plus button)>click Excel
- Select column D (left click), open Find and Replace (Ctrl+F)
- Click on the Replace tab
- Within the find field: *\
- Within the replace field:
- Replace all
- It should say “All done. We made…” If not try again
- Click OK
- Close the Find and Replace box
- Delete Columns not needed, for example columns: A-C, G, I-N, R-W
- Left click and highlight all boxes that you want to sort and filter
- In my example it was from A1 through H271
- Go to Insert>Table
- Click OK
- Click on an arrow next to column A and filter by the Case manager(s) you want to view for oversite
- You can add a notes column next to the last column for it to get added to the table automatically
- Select column D (left click), open Find and Replace (Ctrl+F)
- Group by>Custom Field>Expand Case Manager (plus button)>click Excel

- To add or change assigned direct to client Case Manager or assigned Case Manager to a home
- Contact updates@cfsf.net
- To add or remove assigned BRS Clinical Director OR Temporary Old Case Manager for Up to 2 Weeks
- Contact updates@cfsf.net
- To add or change assigned direct to client Case Manager or assigned Case Manager to a home

- Go to Case Rosters>Foster Care Open Cases
- Open client file
- Click Edit
- Change Case Manager to the receiving care manager, click Save
- Within Extended Reach add the report that was requested for
- Go to the child>Add Report>Click on the missing report name OR Compliance tab>three dots to the right of the report>Add Report
- Confirm “Due:” date is correct
- Change “Where is the file attached to?:” to “Request for Information Attached”
- Upload the email requests as a PDF attachment to this same report.
- Remember that there should be two email requests and one phone request
- For phone requests add Comments within the “Comments” tab of the report
