HSSP
- Once logged in to Extended Reach>Click on the Client from your My Workspace
- Add Report: “HSSP”
- Confirm Due Date is today’s date
- Confirm Submit To is the correct supervisor
- Enter in the relevant information
- Follow the directions in the report, the Save button will allow you to pause even if you haven’t filled in all required fields
- Use the Draft button if you aren’t ready to finish the report
- Once finished click Submit for Approval
- If rejected: you will receive an email, correct HSSP and resubmit for approval
- If approved by the supervisor, you will receive an email, the form is now internally signed and the Next Due Date is added. Follow below instructions for external signatures
- Open the HSSP
- Click on Collect External Signatures
- Confirm email addresses, if any addresses are inaccurate or missing follow below
- For foster parent contact the licensor
- For Social Worker and CASA contact someone with the fiscal department
- If email addresses are correct click on Send
- Confirm email addresses, if any addresses are inaccurate or missing follow below
- The HSSP will be sent by email for signature. Once they sign the document they lose access
- To view outstanding external signatures and remind them to sign
- Open the HSSP
- Scroll to the Collect External Signatures
- Click on Send Reminder / Edit
- Confirm Email and click Send
- To view outstanding external signatures and remind them to sign
- If a needed signature cannot be collected by email follow below
- Print the HSSP. There are two signature boxes on the last page
- Once signed
- Rename document to match policy

- Open the HSSP in Extended Reach
- Click Edit
- Click on the paperclip under Related Files

- Find and click Open to Upload Document

- Click Save
