REQUEST FOR SUPPORT

- Requests for Support should be initiated any time a placement requires additional funding for goods, services, etc
- Before filling out a custom form talk with your Clinical Director
- Once logged in to Extended Reach>Click on the Client from your My Workspace
- Add Report>More>Request for Support
- Submit To: = Program Director
- Fill out all the relevant fields.
- attach any documents, ex: receipts, invoices, etc
- Click on Submit for Approval
- Clinical Director and Program Director receive an automatic email notification
- If rejected: you will receive an email, resubmit if asked to
- If approved by Clinical Director and Program Director, you will receive an email, the form is now internally signed.
- Fiscal will take care of request and will use the Comments section to update progress of the request
- Fiscal will contact you when the request has been completed
